Housing Stabilization Services

Housing Stabilization Services (HSS) is a new benefit for individuals on Medical Assistance to help find and maintain housing.
Housing Stabilization Services has three parts:

Housing Consultation

Assists eligible individuals who do not have access to a case manager or targeted case manager in developing a person-centered plan and submitting it to a Housing Transition provider. Services that assist a person in developing a housing-focused, person-centered plan, assisting the person in obtaining the right state plan resources that support housing stability, and providing referrals or information about other services. People on Medical Assistance who do not have a waiver case manager, mental health targeted case manager, or MSHO/MSC+ care coordinator to conduct an assessment and a person-centered plan of treatment can use this service

Housing Transition

Assists people in preparing for, finding, and moving into their own houses in the community by:

  • Making a house plan that is unique to you
  • Identifying and aiding in the removal of barriers to housing access
  • Assisting the individual in seeking for benefits to help them pay for their accommodation.
  • Getting in touch with potential housing choices to inquire about availability and information
  • Assisting the individual with tenant screening and housing evaluation
  • Assisting in the understanding and negotiation of a lease
  • locating resources to help pay for relocation costs
  • Ensuring that the new living situation is secure and ready.

Housing Sustaining

Supports a person’s ability to remain in their own community home by:

  • Early detection and prevention of behaviors that could risk continued housing
  • Assistance with the re-certification of housing
  • Tenant training, lease compliance, and home management are all available.
  • Supporting the individual in understanding and maintaining their income and benefits in order to keep their housing.
  • Supporting the community’s development of natural housing supports and resources
  • Room and board are not covered by housing sustaining services.

Who We Serve

Eligibility for AlchemyMN Housing Stabilization Services

To be eligible for services, individuals must be:

  • Adults with a mental health condition
  • Receive Medical Assistance
  • Experiencing Housing Instability
    • Currently experiencing homelessness
    • At risk of becoming homeless
    • Transitioning from a licensed setting
    • Eligible for MA waiver services
  • Needs assistance due to their mental
  • health condition to:
    • Communicate
    • Manage challenging behaviors
    • Make decisions

Eligibility for AlchemyMN Individual Home Supports

To be eligible for services, individuals must:

  • Adults with a mental health condition
  • Need Home and Community Based Services
  • Be approved for CADI or BI waiver services

How does the program work?

Housing Stabilization Services provides up to 150 hours per transitional for housing transition and up to 150 hours for housing sustaining per year.
