Ask your county, tribal nation, or health plan for an assessment. It helps your lead agency decide if you qualify and how many hours you can get.

Choose a consultation provider

This is your CFSS guide. They help you pick the agency or budget model and write your plan. Your lead agency can give you a list.

Step 4

Choose your CFSS agency

You can choose any enrolled agency. If you choose us, you can recommend a worker you trust, even a family member.

Your plan is approved

You and your consultation provider write your service plan. Your lead agency reviews and approves it.

Step 6

Care begins at home

Your worker finishes DHS training and a background study, and services start. Your needs are reviewed every year.

What CFSS can help with

The agency is the employer. We hire, train, complete background studies, and handle payroll and paperwork. You still choose who cares for you. Best if you want choice without employer duties.

You are the employer and manage a budget, with help from a financial management services provider. Best if you want full control and are comfortable managing workers.

Common questions

Does CFSS cost me anything?

CFSS is paid by Minnesota health care programs such as Medical Assistance. If you qualify, there is usually no cost to you. Your lead agency can confirm your coverage.

Can my family member be my paid caregiver?

Yes. Under CFSS, eligible family members, including spouses and parents of minor children, can be paid support workers after they finish DHS training and a background study. Parents and spouses can be paid for up to 60 hours a week (80 hours combined if both parents work), at the standard support worker rate.

What is a consultation services provider?

Your CFSS guide. They help you choose a model and write your service plan. They must be a different company from your CFSS agency.

I don’t have an assessment yet. Can you still help?

Yes. Call us and we’ll explain how to request a MnCHOICES assessment from your county, tribal nation, or health plan.

Can I talk with you in my own language?

Yes. We use LanguageLine to set up a free professional interpreter for your calls and visits with AlchemyMN, so you don’t need to arrange it through your case manager or county. For your MnCHOICES assessment, we can help you ask your county or health plan for an interpreter.

I’m on PCA. Do I need to do anything now?

Most people move to CFSS at their next yearly reassessment. Your case manager will reach out. You can also call us with questions anytime.
